I acknowledge that physical activities, including but not limited to hiking, mountaineering, hiking, backpacking, biking, skiing, archery, yoga, stretching, and traveling in mountain terrain, are extremely hazardous, and I am voluntarily participating in these activities with knowledge of the danger involved. I accept any and all risks of injury, death, or damage resulting from or in connection with my participation in and transportation related to these activities.
I indemnify, release, and forever hold harmless the BLM, U.S. Government, and Adventure Rabbi: Synagogue w/o Walls and all of their groups, directors, officers, guides, members, employees, agents, leaders, instructors, contractors, and volunteers, together referred to as "associates," from my personal injury, death, or property damage, whether caused by the negligence of its associates or otherwise as a result of or in connection with my participation in Adventure Rabbi activities or events. This release shall bind all of my heirs and successors. I further this indemnification to any of my children and/or wards in connection with Adventure Rabbi activities or events.
This document is covered by the laws of the State of Colorado, and is intended to be as broad and inclusive as permitted by the laws of the State of Colorado. If any portion of this document is held as not enforceable or valid, the remaining portions shall remain in full legal force and effect.
I understand that trip leaders have complete authority on each trip, and this includes 1) excluding me from participation based on qualifications, condition, clothing, equipment, food, and water; 2) setting and maintaining the pace, varying the route, and turning back the party as warranted. I hereby grant to Adventure Rabbi the irrevocable and unrestricted right to use and publish photographs of me and my children / minors, or in which I may be included, for editorial trade, advertising and any other purpose and in any manner and medium; and to alter the same without restriction. I hereby release photographer and Adventure Rabbi from all claims and liability relating to said photographs. I understand that I am responsible for keeping an eye on my own children and insuring that they stay with the group and behave in a safe manor.
By signing this I state that I have now been advised to wear hiking boots or other proper footwear, to carry water, extra warm clothing, raingear, and a flashlight. I also state that I am in proper physical condition to perform strenuous physical activities at high altitude. Parent or legal guardian must sign for minors.
Please note that registration fees are non-refundable.

CGL TERMS OF AGREEMENT -- Signature required It is important that a participant, parent or legal guardian ("parent") and their families understand our policies. Parents: Please read carefully, review with your children, sign below and contact us if you have questions. Non-discrimination Policy CGL does not discriminate in their respective employment or admissions policies on the basis of race, color, gender, sexual orientation, religion, national or ethnic origin, age or disability. Photo Release By signing this form, you authorize CGL to take and use you or your family’s photo, written statement and/or audio or video recording and use it in any manner it desires, for program, promotional or other use. Right to Dismiss a Participant CGL reserves the right to dismiss any family that staff believes, in their judgment, presents a safety concern or medical risk, is disruptive, or engages in behavior detrimental to the program. If a family is dismissed or departs for any reason, family is responsible for all costs of early departure whether for medical reasons, dismissal, personal emergencies or otherwise. In consideration of the services of Mountain Day Camp, Inc. DBA Camp Granite Lake, and its employees, agents, owners, directors, officers, representatives, independent contractors, and all other persons or entities associated with it, including the owner/s and/or lessor/s of lands on which the activities take place (collectively referred to in this Document as ‘CGL’), I acknowledge and agree as follows: ACKNOWLEDGMENT AND ASSUMPTION OF RISKS Engaging in CGL educational, recreational and/or adventure activities (which may be mandatory or optional, scheduled or unscheduled, and conducted by CGL staff or independent contractors) may include, but not be limited to hiking, backpacking, camping, rock climbing (outdoors on natural rock, or indoors on artificial walls), challenge course and team building activities, kayaking, rafting, horseback riding, sports, sailing, rock throwing on the range, archery, crafts, bar-b-ques and social events, nature and ecology, swimming, ceramics, canoeing, wood working, stand up paddle boarding, orienteering activities, service learning projects, and transportation in vans or other vehicles to and from activities (collectively referred to in this Document as ‘activities’). I acknowledge that these activities include inherent and other risks, hazards and dangers (collectively referred to in this document as 'risks') that can cause injury, damage, death or other loss to my child or others. I agree to discuss the nature of these activities and risks with my child. The following describes some, but not all of those risks: Risks present in an outdoor or wilderness environment. These risks include travel in high altitude, mountainous, or wilderness terrain, both on and off trail. Participants’ travel and activities may be subject to lightning; strong winds; fast moving rivers or other water bodies; falling rocks; extremely hot (geothermal) or cold water or weather; flash floods; snow and ice; fallen timber; venomous, stinging or disease carrying animals or insects; poisonous plants; wild animals and other natural or man-made hazards. Hazards may not be marked or visible and weather is always unpredictable. Risks involved in decision making and conduct, including, without limitation, the risk that a CGL staff member, representative, contractor or co-participant may misjudge a participant’s capabilities, health or physical condition, or misjudge some aspect of instruction, medical treatment, weather, terrain, water level, or, river and/or terrain route location. Personal health and participation risks. The risk that participant’s mental, physical or emotional condition/s or limitation/s (known or unknown, disclosed or undisclosed) combined with participation in these activities could result in injury, damage, death or other loss, including contractable diseases and epidemics. Although CGL personnel may review participant’s health information, CGL cannot anticipate or eliminate risks or complications posed by participant’s mental, physical or emotional condition. Risks associated with cooking and camping. Risks include gas explosion, scalding or other burns associated with cooking over a gas stove or open fire, and water contamination in natural water sources. Water from natural sources should be disinfected, filtered or boiled before use. Risks regarding conduct. The potential that the participant, or other participants or third parties (e.g. driver, rescue squad, hospital) may act carelessly or recklessly. I agree: - CGL representatives are available should I have further questions about these activities or the associated risks; - If a participant has any mental, physical or emotional conditions or limitations that might affect his/her ability to participate I agree to disclose those to CGL, and represent that the participant is fully capable of participating without causing harm to him/herself or others; - CGL cannot assure a participants safety or eliminate any of these risks. Parent/s agree that a child participant is voluntarily participating in all CGL activities with knowledge of the risks. Therefore, participant and parent/s assume and accept full responsibility for participant, for the inherent and other risks (both known and unknown) of these activities and for any injury, damage, death or other loss suffered by participant (and parent/s), resulting from those risks, and/or resulting from participant’s own negligence or other misconduct. RELEASE AND INDEMNITY AGREEMENT Please read carefully. This Release and Indemnity agreement contains a surrender of certain legal rights. I (the participant or participant’s parent) for myself and for and on behalf of my participating minor child, agree as follows: 1) to release and agree not to sue CGL, with respect to any and all claims, liabilities, suits or expenses (including attorneys’ fees and costs) (hereafter collectively 'claim' or 'claim/s') for any injury, damage, death or other loss in any way connected with my child’s enrollment or participation in these activities, or use of any equipment, facilities and/or premises. I understand that in signing this Document, I, my child, and anyone acting on my or my child’s behalf, surrender our respective rights to make a claim against CGL as a result of any injury, damage, death or other loss suffered by me or my child; 2) to defend and indemnify (‘indemnify’ meaning protect by reimbursement or payment) CGL with respect to any and all claim/s: a) brought by or on behalf of me or a family member for any injury, damage, death or other loss in any way connected with my child’s enrollment or participation in these activities, or use of any equipment, facilities and/or premises; and/or b) brought by a co-participant or any other person for any injury, damage, death or other loss to the extent caused by my child’s conduct in the course of participating in these activities, or using any equipment, facilities and/or premises. This Release and Indemnity Agreement includes claims resulting from CGL’s negligence (but not its gross negligence or willful, wanton or criminal misconduct),, and includes claim/s for personal injury or wrongful death (including claim/s related to emergency or medical response, assessment or treatment), property damage, loss of consortium, breach of contract or any other claim. CONCLUSION I agree that the substantive laws of Colorado (without regard to its conflict of laws rules) govern this Document, any dispute I have with CGL and all other aspects of my relationship with CGL, and that any mediation, suit or other proceeding must be filed or entered into only in Boulder County, Colorado. I agree to attempt to settle any dispute (that cannot be settled by discussion) through mediation before a mutually acceptable Colorado mediator. This Document is intended to be interpreted and enforced to the fullest extent allowed by law. If any portion of this Document is deemed unlawful or unenforceable, it shall not affect the remaining provisions, and those remaining provisions shall continue in full force and effect. COVID-19 Notice COVID-19 is a respiratory illness that appears to spread easily from person to person and may be life threatening to those suffering from it. Your family’s health and safety is important to us. For that reason, we want you to be aware that your child will interact in close proximity to and with other children and adults for periods of time while at camp. Although we will maintain proper (and enhanced) hygiene and infection control practices, it is impossible to guarantee that your child (or anyone else at camp) will not become sick or be exposed to COVID-19. By signing this document, you acknowledge your understanding that COVID-19 is spreading rapidly through the United States, that during camp your family will spend time around many children and adults, and that it is possible that your family may become sick with or exposed to COVID-19, whether at camp or elsewhere. Choosing not to agree will disqualify your family from participating in a program at camp.

We urge you to purchase private trip insurance in case you need to cancel. Due to the high expenses of running this trip and the low cost we charge participants, we are unable to refund your payments for any reason, even really good ones. The best we will be able to do is convert your payment into a donation if you let us know in advance that you are not coming. If we cancel the event, you will receive a full refund.
I understand there will be no refunds unless Adventure Rabbi cancels the trip.
