Contract:

This form will serve as a contractual agreement for a ceremony of welcome for your child.

• The Rabbi will create a personalized ceremony for your newest addition.
• Please plan to meet with the Rabbi for 1 hour before the service.  This may be done virtually.
• The Rabbi will bring personalized programs crafted for your service. (If a mohel or mohelet is co-leading, sometimes they create the service program instead.)
• Please provide a kiddush cup with juice or wine, tallit (if you own one), challah, and challah cover.
• You are responsible for arranging and preparing the venue.
• If Rabbi Korngold is flying in, the client is responsible for economy-plus airfare, 2 nights lodging and ground transportation.
