This form will serve as a contractual agreement for rabbinical services:

Fee: $7,500

Includes:

• Officiation by Rabbi Jamie Korngold at personalized service for two students in Mammoth CA on Sat Jun 20, 2020.
• 1-hour meeting to plan the service via videoconferencing
• Rehearsal in person or videoconferencing
• Use of our backpacking Torah
• Electronic version of our prayerbook with permission to print with a personalized cover.
• 4 member-rate tickets for attendance at Passover in Moab
• Students are welcome to schedule 2 meet and greet sessions with Rabbi Jamie. There will be no formal meeting or agenda, just 15-minutes of getting to know each other.
• A complementary hike with Rabbi Jamie in Boulder CO for the family.
• Students will learn Hebrew prayers, select Torah portions (which do not have to be be pegged to a date) and do all of their other education with their tutor.
• Each student will meet with Rabbi Jamie 2-3 times for 30-minute meetings, and collaborate over Googledocs, to craft their speeches

In addition to tuition, the family will be responsible for these costs:

• Travel expenses: airfare w/ economy plus seating, rental car & gas
• Lyft to and from Denver airport
• Food while traveling
• Two nights lodging in a business or resort class hotel
• Printing of prayerbooks

Discounts, Service Fees & Refunds:

-Monthly Payment Plan: Your initial payment of $600 will include a one-time service fee of $100, and a $500 deposit. The remainder will be divided into 7 monthly payments, which are automatically withdrawn using ACH and are non-refundable. A few days after you register, you will recive an email from our office with a form to fill out to enable ACH withdrawals. If you would rather continue to use credit card, you may but there will be a 4% conveneince fee.
– Payment in Full: Today, your card will be charged $500. The remaining $7,000 can either be paid using ACH or credit card. If you would like to use credit card, there will be a 4% conveneince fee on the $7,000. 50% of the full payment will be non-refundable. Four months before the event, the entire amount becomes non-refundable.
